Acknowledgment of Risk & Liability Waiver

By using our services, facilities, or participating in any indoor sports activities (including but not limited to Pickleball, Volleyball, and Basketball), I understand and agree to the following:

1. Assumption of Risk

I acknowledge that participating in indoor sports involves inherent risks, including but not limited to slips, falls, collisions, equipment failure, physical injury, and unforeseen accidents. I understand these risks and voluntarily choose to participate at my own risk.

2. Release of Liability

I hereby release and hold harmless [Company Name], its owners, employees, partners, sponsors, affiliates, and facility providers from any and all claims, demands, damages, or causes of action arising out of or in connection with any injury, illness (including COVID-19 or other contagious conditions), property damage, or loss sustained during my participation.

3. Medical Consent

In the event of a medical emergency, I authorize the staff or representatives to arrange for emergency medical care as necessary. I understand that I am responsible for all related costs.

4. Rules & Conduct

I agree to follow all facility rules, safety instructions, and any additional guidelines provided by staff. I understand that failure to comply may result in the loss of access or removal from the premises.
